travel, taking them out of a hospice's service area for a temporary
period of time. It is important that both the HOME hospice and
the TRAVEL hospice understand their respective responsibilities
to assure a safe and well coordinated trip. This program provides
valuable information for all involved in the process: both hospices,
the patient and their families.
