Description:
This module provides basic information on the Local Coverage Determination ("LCD") guidelines, and provides a detailed explanation of the criteria required for hospice admission and recertification for patients with a dementia diagnosis.

Objectives:

• Participant will Define the phrase Additional Development Request ("ADR").
• Participant will Prepare a medical record for initial ADR submission.
• Participant will Identify appropriate documentation to support patient eligibility.
• Participant will Discuss proactive strategies to improve visit note and other documentation.
