Session 1 - Introduction to QAPI

This session is an introduction to the QAPI Condition of Participation and its standards covering topics such as assessment and care planning, patient-level and hospice-level quality assessment and the role of the governing body in QAPI.
Session 2 - Program Data and Performance Measures

The viewer is introduced to the requirements of the program data standard of the QAPI Condition of Participation. The speaker discusses data elements and how they are used to construct quality indicators and introduces the concept of a framework for selecting and organizing quality measurement information. Practical examples for use in hospices are provided.
Session 3 - Program Activities

Session 4 - Performance Improvement

This QAPI session describes the specific requirements of the performance improvement standard of the QAPI regulation including the critical elements of performance improvement projects.
Session 5 - Patient-level and Hospice-level QAPI Measures

Understanding the value of both patient-level and hospice-level quality measures is essential to an effective QAPI program. The session explains how collecting data on the experiences of an individual patient helps to improve care for that patient while aggregated data collected in both clinical and non-clinical areas helps to improve operations across the hospice program.

The QAPI Condition of Participation comes with both challenges and opportunities for hospices as they implement the various components of a QAPI program. This presentation from a former QAPI Coordinator actively engaged on the QAPI frontlines walks you through identifying your stakeholders who are instrumental in implementing a successful QAPI program; sharing strategies for creating an organizational "culture of quality"; discovering existing QAPI national and state-level benchmarking tools; and delineating a plan for conducting successful Performance Improvement Projects (PIPs).

Melanie P. Merriman, founder of Touchstone Consulting in North Bay Village, FL, provides clients with quality measurement, market research, strategic planning, and program evaluation services with a focus on chronic illness and end-of-life care. Dr. Merriman is also the project director for EDELE (Epidemiology of Death and End of Life Experience), a federally-funded initiative to provide Internet access to public data for healthcare providers and policy-makers. From 1992-1998, Dr. Merriman was Director of Quality and Compliance for VITAS Healthcare Corporation. She is also a co-author of the Missoula-VITAS Quality of Life Index. Dr. Merriman earned a Ph.D. in Biology and an MBA in Health Care Administration. She is a co-author of The QAPI Requirement: Resources for Hospice Programs.
Heather P. Wilson, President of Weatherbee Resources, Inc. and the Hospice Education Network is the author or co-author of numerous hospice regulatory manuals including most recently, CoPs in a Binder: The Interim Edition and The QAPI Requirement: Resources for Hospice Programs. Dr. Wilson is a member of NHPCO's Regulatory Committee and was leader of its Education Workgroup for 6 years. She is a member of the Board of Directors of the Hospice and Palliative Care Federation of Massachusetts and a frequent speaker at State and national hospice conferences. She is a permanent faculty member of Weatherbee's Hospice Regulatory Boot Camps. Dr. Wilson received her Ph.D. in Pastoral Psychology from Boston University.
C.Andrew Martin is the Director of Marketing and Special Projects for Weatherbee Resources, Inc. Prior to joining Weatherbee Resources, he was the Education and QAPI Coordinator for Seacoast Hospice and responsible for preparing the hospice to meet the QAPI requirements. Mr. Martin is an End of Life Nursing Consortium (ELNEC) trainer and a HPNA-Approved Clinical Review Educator and been an adjunct faculty member at the University of New Hampshire. Mr. Martin is a frequent presenter at State and national hospice conferences and for the Hospice Education Network.
